MEDICARE BILLED SERVICE Chronic Disease Management Plans Chronic care plans have been developed by the Government of Australia to provide Allied Health services, including chiropractic care, as an approved health service. Patients with a Chronic Diseases (including musculoskeletal pain and/or mental health illness) are eligible for five (5) Allied Health Professional services per year funded by Medicare. The CDM program is by referral only and needs to be planned through a GP. Each patient is able to utilise the 5 treatments for Chiropractic care or in combination with others Allied Health services as decided by individual needs and in discussion with the GP. At least 3 treatments are required in order to diagnosis and complete a 'trial of treatment' as most patients under this program have chronic conditions that have been long standing. It is also that patients choice as to which chiropractor they receive treatment, although the GP may recommend other services. The Medical Rooms can fax through the referral to us or the paperwork can be brought into our Centre in person prior to the Initial Appointment. The referral needs to include any information with regards to the patients complaint(s) and to include: Diagnostic Imaging reports or films (plain films / CT / MRI) and/or blood tests or other diagnostic test results and medications. Once lodged with Medicare by the GP, we require 2 business days to process the paperwork prior to booking an appointment. On the day of the appointment patients will need to bring their Medicare card and we do require forms to be completed for our records, so please allow time to do this. Patients may bring 1 support person if assistance is require with forms, mobility or if English is a second language. Missed appointment will incur a fee. If a Medicare Chronic Disease Plan has recently been completed call us and we will set up appointments so that the plan does not lapset lapse. Department of Veterans' Affairs If you have a Repatriation Health Card, DVA will fund for chiropractic services on referral from your GP NOT ALL REGISTERED CHIROPRACTORS PARTICIPATE IN THESE MEDICARE FUNDED PROGRAMS For information about eligibility for either Medicare or DVA please contact our Front Desk Staff
3rd PARTY PAYERS WorkCover & Motor Vehicle Accident We accept WorkCover & Motor Vehicle Accident patients on a Claim by Claim basis. In order to consider a Claim we require confirmation from a Claims Agent that your claim has been approved. We DO NOT bill 3rd parties ourselves. You will be required to claim your re-imbursement via the 3rd party.
